Dear Parent/Guardian:
Welcome back to the 2024-2025 school year. I am excited to be working with the great staff here in
Stanfield. My name is Bryn Braithwaite, BSN NCSN RN, and I am the school nurse for Stanfield
School District. My goal is to help all students to stay healthy and learn. The following information
includes guidelines Stanfield School District follows regarding your child’s health.
• Please DO NOT SEND AN ILL STUDENT TO SCHOOL. The back side of this letter gives
examples of when your student should not be in school based on their symptoms. If you
have any questions, please contact the school office or me, and we will provide guidance.
• IF YOUR STUDENT IS ILL, PLEASE NOTIFY THE OFFICE.
• Please notify the school if your child is diagnosed with a CONTAGIOUS DISEASE,
including these: chickenpox, diarrhea caused by E.coli or Salmonella or Shigella, hepatitis,
measles, mumps, pertussis, rubella, scabies, tuberculosis, or another disease as
requested. The school will protect your private information as required by law. [OAR 333-
019-0010; ORS 433.008.]
• Please contact your health care provider about any SERIOUS ILLNESS or if you are
worried about your student’s health. If you need help finding a health care provider, you may
contact the local public health authority, or I can also assist you in this process.
• If an antibiotic is prescribed, the student must be on the ANTIBIOTIC for at least 24 hours
before returning to school.
• Please notify the school if your student requires MEDICATIONS during school hours.
Paperwork will need to be completed at the office and a designated time confirmed to drop
off medication. Students under the age of 15 MAY NOT bring in their own medication. If you
have any questions about medications at school, please contact me at the number below.
Currently, we do not allow any student to self-carry medication at the elementary level. If
your student plans to self-carry medication at the secondary level, please complete a self-
carry form in the office.
• Please notify the school if your student has an UNDERLYING OR CHRONIC HEALTH
CONDITION. We will work with you to address the health condition and support the student
in their learning. With consent, the school nurse may consult with the student’s health care
provider about the health condition and necessary treatments and accommodations.
As the school nurse, I want to support your students in their health and education. Please feel free
